First-in-Man Implantation of a Tricuspid Annular Remodeling Device for Functional Tricuspid Regurgitation.

چکیده
T ricuspid regurgitation (TR) is a common finding in patients with left-sided disease, and is associated with poor outcome and predicts poor survival, heart failure, and reduced functional capacity (1). It is common after mitral valve treatment in rheumatic or ischemic mitral regurgitation, if left untreated at the time of mitral surgery. In this subgroup with long-standing TR, managed medically for a long time, surgical correction can be more prohibitive due to the presence of variable degrees of right ventricular (RV) dysfunction, pulmonary vascular disease, and right heart failure. The pre-operative condition of the RV and the severity of secondary renal and hepatic impairment are predictors of limited survival (2,3). Percutaneous procedures may be an attractive alternative to surgery for patients deemed to be high-risk surgical candidates. While the development and clinical use of percutaneous approaches to the aortic valve and mitral valve have been widespread over the past few years, limited data are available about the feasibility and efficacy of percutaneous tricuspid valve (TV) therapies (4,5).

In vitro characterization of the mechanisms responsible for functional tricuspid regurgitation.
BACKGROUND Functional tricuspid regurgitation (TR) is increasingly recognized as a source of morbidity. Current repair strategies focus on annular remodeling because annular dilatation is common in patients with TR. Although papillary muscle (PM) displacement is recognized in functional mitral regurgitation, its role in TR is less well characterized. The objective of this in vitro study was to ...
متن کاملClinical and Echocardiographic Factors Affecting Tricuspid Regurgitation Severity in the Patients with Lone Atrial Fibrillation
BACKGROUND Atrial fibrillation (AF) can be a risk factor for development of significant tricuspid regurgitation (TR). We investigated which clinical and echocardiographic parameters were related to severity of functional TR in patients with lone AF. METHODS A total of 89 patients with lone AF were enrolled (75 ± 11 years; 48% male): 13 patients with severe TR, 36 patients with moderate TR, an...
متن کاملLeft ventricle restoration in end stage ischemic dilated cardiomyopathy
Background Patients with end stage dilated cardiomyopathy exhibit extensive remodeling of the left ventricle, annular dilation, and significant mitral and tricuspid regurgitation. These changes increase perioperative morbidity and mortality, and emphasize patient candidacy for heart transplantation. We previously published that Reductive Annuloplasty of Double (mitral and tricuspid) Orifices co...
متن کاملTricuspid valve repair for treatment and prevention of secondary tricuspid regurgitation in patients undergoing mitral valve surgery.
Secondary or functional tricuspid regurgitation occurring late after mitral valve surgery is associated with high morbidity and mortality. In this article, we review the pathophysiology of secondary tricuspid regurgitation and the evidence supporting the use of tricuspid valve annuloplasty for preventing and treating secondary tricuspid regurgitation. Liberal application of tricuspid valve annu...
متن کاملSurgical management of functional tricuspid regurgitation with a new remodeling annuloplasty ring.
BACKGROUND Moderate-to-severe functional tricuspid regurgitation (TR) should be corrected in patients undergoing surgery for left-sided valvular diseases, to improve long-term outcomes. Several techniques of surgical repair (suture annuloplasty or prosthetic annuloplasty) to correct this condition have been described. Multiple clinical studies have shown the superiority of prosthetic remodeling...
